This handbook is provided to individuals admitted to services through The Crossroads as part of the intake and orientation process for our Psychiatric Rehabilitation Program (PRP).
The handbook contains important information regarding:
- Program expectations
- Participant rights and responsibilities
- Confidentiality and privacy practices
- Grievance procedures
- Attendance and participation expectations
- Available support services and community resources
Individuals receiving services are encouraged to review the handbook carefully and discuss any questions or concerns with program staff.

Our Notice of Privacy Practices explains how protected health information may be used and disclosed, as well as your rights regarding your healthcare information.
By completing the acknowledgment section below, you confirm that you:
- Received access to the PRP Orientation and Policies Handbook
- Were offered a copy of the Notice of Privacy Practices
- Understand that questions regarding policies, procedures, or services may be discussed with program staff at any time
